Our assessment
Read the two figures together. A single troche contains 40mg of tadalafil. The approved maximum on-demand dose of tadalafil is 20mg. The instruction is to take half a troche.
So the intended dose is defensible — 20mg on-demand is exactly the approved ceiling — but it is only reached if the patient correctly halves a lozenge every time. Take a whole one, whether by misreading the label, by assuming a unit is a dose, or because it is dark and you are in a hurry, and you have taken double the approved maximum.
That is a design decision worth naming. Approved products handle this by putting one dose in one unit, which is why tablets come in 5, 10 and 20mg rather than in 40mg tablets you are trusted to bisect. Placing a 2× dose in a single lozenge shifts a safety margin from the manufacturing process onto the patient, at the moment he is least likely to be reading carefully.
The apomorphine is the same 6mg-per-troche arithmetic — 3mg intended. Credit where it is due on transparency. Empower states its regulatory basis, names its section of the Act, publishes exact strengths, and says plainly that safety and efficacy have not been evaluated by the FDA.
