Open to legal residents of the fifty United States and the District of Columbia who are 18 years of age or older at the time of entry. Void where prohibited or restricted by law.

Employees of the Sponsor and of CoreAge Rx, and members of their immediate households, are not eligible.

Because the prize is a prescription product, the winner must be able to complete CoreAge Rx’s own medical consultation and be approved by a licensed prescriber. Nothing in these rules guarantees that any entrant will be prescribed anything. If the winner is not approved, or declines to complete the consultation, the prize will be forfeited and an alternate winner may be drawn.

Entries are accepted from 2 August 2026 until 30 September 2026, 11:59pm Central Time. The Sponsor’s clock is the official timekeeper.

Submit your email address using any of the entry forms on this site. One entry per person and per email address for the entry period; duplicate entries are consolidated to one.

Free method of entry. No purchase is necessary. If you prefer not to submit a form, send a postcard bearing your name, email address and the words “4PLAY Giveaway” to the Sponsor at the address above, to arrive before the entry period closes. Postcard entries receive the same weight as online entries.

One (1) prize: A 12-month CoreAge Rx 4PLAY subscription — twelve consecutive monthly fulfilments of CoreAge Rx 4PLAY, awarded as prepayment or reimbursement of the subscription at $114 / month.

Approximate retail value: $1,368, being twelve months at CoreAge Rx’s published price at the time these rules were issued. The prize has no cash alternative and is not transferable. The Sponsor may substitute a prize of equal or greater value if the advertised prize becomes unavailable.

The prize does not include, and the Sponsor does not pay, any consultation fee, shipping charge, or any cost of medical care.

Odds of winning depend on the number of eligible entries received. One winner will be selected at random from all eligible entries by 7 October 2026.

The winner will be notified at the email address entered. If the winner does not respond within fourteen days, the prize is forfeited and an alternate winner may be drawn. Every entrant will be emailed the result either way.

The winner is responsible for all federal, state and local taxes arising from the prize. Where the value requires it, the winner may be asked to provide a completed IRS Form W-9 before the prize is awarded.

Entering adds your email address to our mailing list. We collect your email address and nothing else — no symptom, medication or other clinical information is requested or stored by the entry form.

You may unsubscribe at any time using the link in any email, and unsubscribing does not remove you from the draw. Our privacy notice explains how we handle the address.

The Sponsor may disqualify any entrant who tampers with the entry process, and may cancel, suspend or modify the promotion if fraud or a technical failure makes it unworkable, awarding the prize from eligible entries received to that point.

By entering, you agree to these rules and to the decisions of the Sponsor, which are final. These rules are governed by the laws of the State of Texas, without regard to conflict of law rules.
