Our assessment

Sildenafil and tadalafil have been generic for years. They are the exact molecules studied in the trials that made PDE5 inhibitors first-line therapy, manufactured to FDA-approved standards, and dispensed at doses a prescriber can titrate deliberately.

This matters as a reference point. Much of the direct-to-consumer market is selling compounded reformulations of these same two molecules at a monthly subscription price, and the marketing rarely mentions that the approved version exists and is frequently cheaper.

The catch is that it requires a prescriber, and the whole appeal of the online model is not having to talk to one. That is a real convenience, and it is also precisely where the clinical value sits.

Reasonable for

  • Almost every man starting treatment for erectile dysfunction
  • Men who want a dose their prescriber can adjust methodically
  • Men who would rather not be on a recurring plan

Not for

  • Men who have had a genuine maximum-dose trial without benefit — that finding points to second-line therapy, not a different tablet
