Our assessment

Judged as a compounding pharmacy rather than a direct-to-consumer brand, this is largely how it should be done. A prescription is required, so a clinician has made the decision. The compounding disclosure is stated in the company’s own plain words. And the safety information published here is the most complete of anything reviewed on this page — nitrates and riociguat both named, the cardiac and ophthalmic contraindications listed, and priapism identified as an emergency.

Set that against the direct-to-consumer compounded products further down this page, several of which publish multiplier claims and no contraindications at all. Same regulatory category, entirely different conduct.

The strengths are where we would raise a hand. 17, 36, 60 and 80mg are unusual numbers but all sit under sildenafil’s approved 100mg ceiling, and a prescriber choosing a non-standard dose for a particular patient is exactly what compounding is for. 115mg is different — it is above the maximum approved single dose, and a prescriber ordering it should be able to say why, because the evidence that made sildenafil first-line therapy stops at 100.

The onset claim of 15 to 30 minutes also deserves the same scrutiny we have applied elsewhere. The best randomised evidence comparing a dissolving sildenafil against ordinary tablets found a difference of about seven minutes that did not reach statistical significance, with onset in both arms closer to fifty minutes. We have written that up separately; it is worth reading before choosing a format on speed.
