Our assessment
Start with what this gets right, because it is more than most of this category manages. Sildenafil 60mg is below the 100mg ceiling. Tadalafil 10mg is a standard on-demand dose, well under the 20mg maximum. Neither figure is a stunt. The compounding disclosure is stated in full rather than buried, and the default is a one-time purchase rather than a subscription you have to remember to cancel. The problem is the premise, not the arithmetic. Sildenafil and tadalafil are both PDE5 inhibitors. They work on the same enzyme by the same mechanism, and combining two drugs of one class does not usually create a new capability — it reproduces the same effect while stacking the risks.
Georgia Urology, a large urology practice, puts the clinical view plainly: it is “unlikely there will be any substantial difference between a single and dual medication therapy,” and “by taking a combination of drugs, patients are increasing their risk of experiencing a side effect — they are now subjected to the risk profile of two drugs rather than just one.” Their conclusion on the category is that it “may be marketed as effective, but ultimately, they may be a waste of money.”
Then there is a smaller thing that tells you something. The product is sold as a lozenge and the instruction says to take one capsule orally. Those are different routes. A lozenge dissolved in the mouth can in principle absorb across the mucosa; a capsule swallowed whole cannot, and travels through gut and liver exactly as a tablet does. The fifteen-minute onset claim only has a mechanism behind it under the first reading, and the instructions describe the second.
The duration claim has the same slippage in a milder form. “Remains active for up to 36 hours” is a property of tadalafil, and a real one — but it is what tadalafil does on its own, at a dollar a tablet. The sildenafil contributes nothing to it.
Which frames the price. You are paying a large multiple for a combination whose benefit over either component alone has not been demonstrated.
