The demand is enormous and entirely understandable. A prescription means a conversation, a conversation means saying it out loud, and a large number of men would rather buy something from a shelf or a website that asks nothing of them. Search volume for non-prescription ED pills dwarfs search volume for most of what a urology clinic actually does.
So this article is not going to lecture you for wanting one. It is going to explain what is in the category, because the honest answer is more interesting than either of the two stories usually told about it.
The uncomfortable finding
The common assumption is that male enhancement supplements are inert — herbs and vitamins that do nothing. That would be reassuring. It is frequently not what regulators find.
Laboratory analysis of seized and sampled products in this category has repeatedly found undeclared prescription drugs, most often sildenafil or tadalafil or close analogues of them, present at unpredictable and undisclosed doses. This is a documented and recurring pattern, not an isolated scandal.
Which produces a strange situation. Some of these products work — and they work because they contain the same molecule a physician would have prescribed, in an amount nobody has told you, manufactured by somebody nobody has inspected.
If a supplement reliably produces an erection, the most likely explanation is that it is a prescription drug wearing a different label.
Why that specifically matters
The danger here is not abstract, and it has nothing to do with disapproving of supplements. It is a drug interaction problem.
PDE5 inhibitors are absolutely contraindicated with nitrates — nitroglycerin, isosorbide, and the recreational alkyl nitrites sold as poppers. The combination can produce a catastrophic drop in blood pressure. A man on a nitrate for angina knows not to take Viagra. He does not know not to take a herbal supplement, because nobody told him it contained a PDE5 inhibitor. Nothing on the label warned him, because the label does not admit what is in it.
The same applies, less dramatically, to men on alpha-blockers, men with unstable cardiac disease, and men taking medications that affect how these drugs are cleared by the liver. The entire safety architecture around this drug class depends on knowing you are taking it.
What about the ingredients that are declared?
The named botanicals in these products — horny goat weed, maca, tribulus, ginseng, yohimbe — have a mixed and generally thin evidence base for erectile dysfunction. Some have small studies suggesting modest effects. None approaches the evidence behind PDE5 inhibitors, and none is standardised in the way a prescription medicine is.
L-arginine and L-citrulline are worth treating separately because the mechanism is real: both are precursors in the nitric oxide pathway. Trials of L-citrulline in erectile dysfunction have used doses around 1.5 grams daily and reported modest benefit. The relevant detail is dose — several products we have looked at while reviewing this market contain a small fraction of that, which makes the ingredient list accurate and the effect negligible.
Yohimbine deserves a specific caution. It is pharmacologically active, it does have some historical evidence in erectile dysfunction, and it can raise blood pressure and heart rate and cause anxiety. It is not a benign herb, and it interacts with a number of medications.
There is exactly one legitimate exception
Everything above concerns supplements. It would be wrong to leave the impression that nothing sold without a prescription is above board, because one product is — and it is worth knowing about precisely because it is so rarely mentioned alongside the rest.
Eroxon, also called MED3000, was authorised by the FDA in June 2023 for over-the-counter sale to treat erectile dysfunction in men aged 22 and over. It is a gel applied to the head of the penis immediately before intercourse.
What makes it unusual is that it contains no drug. The gel is alcohol and water; it evaporates, cooling the glans rapidly and then rewarming it, and that thermal change stimulates nerve endings. Because there is no active pharmaceutical ingredient, it was authorised as a medical device under the FDA’s De Novo pathway rather than approved as a medicine — a distinction that gets reported loosely and is worth holding onto.
It contains no drug, which is exactly why it matters: it cannot interact with nitrates.
That consequence is the genuinely valuable one. A man taking nitrates for angina cannot take sildenafil, tadalafil, vardenafil, avanafil or any compounded combination of them — the interaction can cause a catastrophic drop in blood pressure, and the contraindication is absolute. Historically his options were a vacuum device or injection therapy. A topical product with no drug in it does not create that interaction.
Hold the effect size loosely, though. The main comparator study was unblinded and enrolled 96 men, and unblinding matters more than usual here because the mechanism is a sensation the user can feel. Expectation effects in erectile dysfunction trials are substantial. It is an authorised product with real evidence behind it and a narrower demonstrated benefit than a PDE5 inhibitor at a proper dose.
The thing that undercuts the whole category
Here is the argument we find most persuasive, and it is economic rather than moralistic.
Generic sildenafil and tadalafil have been off-patent for years. At a large retail pharmacy, generic tadalafil runs around a dollar a tablet, and with a discount coupon thirty tablets can cost between eight and thirty dollars. Many supplement products cost more than that per month.
So the trade you are making is not cheap-and-unproven against expensive-and-proven. It is frequently expensive-and-unproven against cheap-and-proven. The only thing the supplement genuinely saves you is the conversation.
And the conversation is the valuable part
We would say this even if we did not run a clinic. Erectile dysfunction is frequently the first symptom of vascular disease, and it can also point at diabetes, low testosterone, sleep apnoea or a medication side effect. Every one of those is worth finding. A supplement that produces an adequate erection while leaving the cause unexamined has not solved your problem, it has hidden it.

If you are going to buy one anyway
Some men will, and we would rather they did it with the relevant facts. Do not take any product in this category if you use nitrates in any form, including poppers. Do not assume a plant-based label means nothing pharmacological is present. Be sceptical of any product that reliably and rapidly produces an erection, because that is the signature of an undeclared drug rather than a herb. And if you are on medication for blood pressure or your heart, take the product itself to a pharmacist and ask.
Better still: the approved version of this molecule is cheaper than most of what you are considering, and getting it legitimately now takes a two-minute online consultation rather than a difficult conversation in a waiting room. The barrier that made non-prescription products attractive has largely been dismantled.
